Menu/Order of Service/table numbers Order form
These new menus, Orders of service and table numbers are designed to match an A3 Black Mouse tableplan.
They can also be ordered alone for a simple streamlined design.

Option 1 - flat printed (no metallics)
203mm x 150mm textured card in ivory or white for menu or Order of Service
144mm x 144mm textured tent card in ivory or white for table numbers
Flat printed with a rectangular coloured border and your text.
Black text on a single white paper insert inside (40p per additional insert required)

Option 2 - 3D panel with metallic gold or silver border
203mm x 150mm textured card in ivory or white for menu or Order of Service
144mm x 144mm textured tent card in ivory or white for table numbers
Rectangular panel printed with your text and mounted on gold or silver card 2mm off the surface.
Black text on a single white paper insert inside (40p per additional insert required)

Minimum 4 weeks required from date of order (to allow for proofing)

1. Fill in the form below to begin the process. Please ensure all boxes are completed.
2. Either print off the form or press the 'send form' button.
3. Send your payment to Black Mouse, either with the form below or with your name and address on the back of the cheque.
   Alternatively submit the form online and request to pay online or call us with your card details.
4. Black Mouse will send you an email to confirm your details and send a secure online payment link.
5. Black Mouse will begin your covers and insert text layouts and email proofs for approval.
6. Once you are happy with the text proofs they will be completed and inserted into your covers ready to be sent to you.

Communication is primarily via email and proofs etc will be sent this way. For UK orders please allow a minimum of 4 weeks for design and production.
Overseas order will require longer.
You can just call or email us if you wish and we'll advise you. No problem.


Payments
We can accept payment by the following methods.
By credit card or debit card over the phone on 0115 9559137or use our postable card payment form
You can also pay with credit/debit cards online. Email us and we'll tell you how to do it.
If you prefer, pay by cheque, postal order or bank transfer in GBP (Pounds Sterling only) made out to Black Mouse Ltd.

Please note
Your order is not confirmed and no work will commence until the appropriate order form and payment has been received by Black Mouse. You may therefore cancel for a full refund at any time prior to this date.
Your design payment will be forfeited if you cancel after work has begun.
Delivery is not included in any prices and is charged separately. Standard delivery is CANNOT be guaranteed. We STRONGLY recommend you use the guaranteed rapid option if your order is valuable or urgent.
All items remain the property of Black Mouse until the account is settled in full.
Changes while a production order is in progress may incur extra charges and prolong the production time.

Please send any cheques or postal orders made out to Black Mouse Ltd to the address above. Full payment (including postage) is required to secure your order.
